Our Care Program

Dr. Kromhout and his team have a passion for delivering dental health care with integrity and enthusiasm. In an effort to encourage accessible oral healthcare, we have developed an “In-Office” Dental Savings Program available to patients who cannot obtain dental insurance or find that our plan is more cost effective than their current dental coverage plan.

Annual Membership Fees and Benefits:
Single Member: $400
Each Additional Family Member: $350

• 2 exams
• 1 set of digital bitewing x-rays
• 2 routine cleanings
• 2 fluoride treatments for children up to age 15
• Panoramic x-ray (every 3 years)
• Additional services will be subject to a 15% savings
• Crowns, Bridges, Fillings, Root Canals, Extractions, Partials and Dentures, Sealants, etc.

Membership Details:

• 12 month membership fee is due in full upon joining.
• Payments are due on the date of service to receive the savings. This excludes financing options such as Care Credit.
• No other reduction of fees can be combined with membership.
• It is the sole responsibility of the member to maximize their benefits by arranging all appropriate appointments within the 12 month period. If the appointments are not used, the member will not be entitled to a refund.
• Plan will not be carried over to the new year.
• Program is non-transferrable.
• Annual enrollment fee is non-refundable.
